WW Rehabilitative Care System Streams of Care

We are reorganizing the Waterloo Wellington Rehabilitative Care System around four condition-specific streams of care.

Each of these streams responsible for developing evidence-based integrated care pathways to support the delivery of best practice rehabilitative care across the continuum of care (ie. acute » Inpatient Rehab » CCAC » Outpatient Rehab » Community)

The five streams of care are as follows.

Stream Prioritized conditions/initial focus areas of care path
Musculoskeletal (MSK) MSK total hip arthroplasty, total knee arthroplasty, fractured hip
CardioPulmonary (CP) Congestive heart failure, chronic obstructive pulmonary disease
Frail Elderly/Medically Complex (FE/MC) Comprehensive geriatric assessment with a rehabilitative focus, amputation
Stroke/Neurology Stroke (ischemic and haemorrhagic)
Acquired Brain Injury (ABI) Implement ABI best practices
